What is new

• Bypass both cache reads and writes for excluded requests in regular and flexible caching, preventing cached full-page responses from being served to Livewire or Inertia requests.
• Add integration coverage for cold and warm caches, case-insensitive and empty headers, custom exclusions, and disabling exclusions.
• Update test tooling to Pest 5 / PHPUnit 13 for Laravel 13, while retaining the latest compatible Pest 4 for Laravel 12. Both are tested on PHP 8.4 and 8.5.

No public API breaking changes and no supported PHP or Laravel versions removed. Requests with excluded headers now intentionally bypass caching. Applications opting back in should use a custom hasher when headers affect response content.
