exact_new_set says the save added these records and nothing else. A colleague registering a different patient in the same window refuted it, though the frame's audit attribution names the colleague. That was a false alarm, concurrent_new_patient in the independent verification suite.

Now, after the frame judge, a refuted exact_new_set is judged again without the new records the frame attributes to another actor. Only attributed additions are set aside; an unattributed one still refutes. Without attribution nothing changes.

Tests: tests/test_exact_new_set_attribution.py (SQLite, real verify-run). A colleague's logged patient verifies; the same patient with no audit entry is reconciliation_required.

Draft until the live OpenEMR Phase 0 run.
