S2 of the file-write safety series (plan: easonLiangWorldedtech/Zoo-Code#33), part of epic #1375. Stacked on S1 (#1383, version token). Introduces the per-task file observation registry (A2): when the agent reads an existing file, the on-disk version token is recorded against the task. The S4 guarded-write will later compare the recorded observation with the token recomputed before a write to detect "the file changed since the read" (stale) or "the file was replaced" (identity change). This PR records observations only — it does not consult them, so behavior is unchanged.
Changes
src/core/task/observationRegistry.ts (new): ObservationRegistry — an in-memory Map<absolutePath, FileObservation> where FileObservation = { version: string, observedAt: number }; observe replaces on re-observation; plus get/has/clear/size. Pure in-memory, zero I/O, no dependencies.
src/core/task/Task.ts: each Task owns an observationRegistry instance — parent and subtask observations are independent by construction.
src/core/tools/ReadFileTool.ts: after a successful read of an existing file, records computeVersionToken(absolutePath) (S1) into the task's registry. A stat failure never fails the read — the token is best-effort (.catch(() => undefined)).
Tests
New registry spec: observe/get/replace-on-reobserve/has/clear/size semantics.
ReadFileTool spec: reading an existing file registers an observation with the exact on-disk version format; reading an absent file leaves the registry at size 0; subtask isolation (parent task's registry untouched by a subtask's reads).
Review-gate re-trigger (2026-08-30): empty commit a00eef8 (no code change) re-runs CI and CodeRabbit current-head review under the org new PR review gate; the code head remains 2965ad1.
Review feedback (2026-09-14 CodeRabbit cycle):6b821b2 strengthens both version-token-failure regression tests to assert the read content, not just the path (native: "content"; legacy: "legacy content" alongside the existing path checks). The legacy test additionally points the module-level readWithSlice mock at the legacy content — the legacy path slices the raw read through readWithSlice (ReadFileTool.ts:797) and the mock's beforeEach default ("1 | test content") would otherwise mask what the read produced.
Mutation gate: the branch now contains current upstream/main — the head commit is a merge with current main (7328cbf) as its first parent, the same shape as the CI job's synthetic merge commit. The gate's resolvePullRequestBase resolves a merge-commit head to its first parent, and this branch's last main sync (ba46d1f) predates the gate rewrite, so without this merge every CI run would measure the true PR delta plus every main commit since. With current main in the first-parent position, the gate measures the true PR delta (src/core/task/observationRegistry.ts new file, Task.ts + ReadFileTool.ts changes):
Local preflight on the true delta (7328cbf → 5a4e27e): extension: 8 valid / 8 killed / 0 timeout / 0 survived / 0 noCoverage — PASS (exit 0, digest not stale).
Why the merge of main needed no conflict resolution. This head merges upstream/main e943530c8, a 1138-file delta over our base 4f1dee18e. Only four paths intersect that delta: src/core/task/Task.ts, src/core/task/__tests__/Task.spec.ts, src/core/tools/ReadFileTool.ts, and src/core/tools/__tests__/readFileTool.spec.ts. Every other file this unit touches is a path main has not modified in that window, which is why the content merge is clean rather than merely resolved - git merge-tree --write-tree predicted tree 4b7828c03 with no conflict output before the merge was run.
Observation keys are absolute resolved paths, and that is a Windows-shaped detail. The tool records the file it read under the resolved absolute path, not the relative spelling the caller passed. On Windows the resolved key carries backslashes and drive-relative components, so an assertion that the recorded path merely containsexisting.ts is satisfied by the relative spelling too - it cannot tell the two spellings apart on this platform. The observe assertions now compare against path.resolve("/test/workspace", "existing.ts") (and the legacy.ts equivalents), which is the form that actually distinguishes a correctly keyed entry from a mis-keyed one. This is also why the case reads Windows-specific to a POSIX reader: on a POSIX path the two spellings differ more visibly, so the weaker assertion hides less.
A red on this head that predates the current work.pnpm test:solheim-smoke-ci fails locally on this merged tree. It was tested by reverting the three files this round touched (src/core/tools/ReadFileTool.ts, src/core/tools/__tests__/readFileTool.spec.ts, src/core/task/__tests__/observationRegistry.spec.ts) back to the merge commit's own content and re-running: the same two assertions fail in exactly the same way with none of the current changes present, so that failure belongs to the merge state and not to this round's edits. Everything this round touched measures green locally: readFileTool.spec.ts and observationRegistry.spec.ts 102 passed, Task.spec.ts 201 passed, check-types exit 0 (11/11 in the pre-push task graph), root lint exit 0, and the lint-suppressions counts on the touched files are unchanged from the merge baseline. The version-token ordering fix is pinned by a test that mutates the file inside the read itself: with the stat moved back after the read it reddens (expected 1:2:300:4000000000:5000000000, received 1:2:300:9000000000:5000000000), and the implementation was restored from a saved copy rather than a checkout.
…oo-Code-Org#1375)
Introduces the version token - dev:ino:size:mtimeNs:ctimeNs derived from a single fs.stat - a pure function of a file's on-disk state that every process computing from the same state agrees on. The compare-and-swap write guard (A2/A3) will compare the token observed at read time against the token recomputed before a write to detect stale or replaced files. No production callers yet: this is infrastructure for the file-write safety series (plan: #33), part of upstream epic Zoo-Code-Org#1375.
…oo-Code-Org#1375)
Review finding: 'ino is an exact integer' was overstated. Node exposes ino as a float64 number: exact for small POSIX inode numbers, but on modern Windows the file ID exceeds 2^53 so Node's own value is already rounded (verified on node v25: non-zero ino, isSafeInteger=false). It remains deterministic per file (same file -> same token), so the token contract is unchanged; change detection rests on exact dev/size plus the mtime/ctime ns fields. Document the bound instead of claiming exactness.
Zoo-Code-Org#1375)
CodeRabbit finding on this PR: the default numeric fs.stat() loses precision (values above 2^53 are rounded, including Windows file IDs) and the ms->ns derivation introduced a double-precision quantum. Fixed by fetching the stat with { bigint: true }: all five token fields (dev, ino, size, mtimeNs, ctimeNs) are exact BigInt values rendered as decimal strings, with no float anywhere. The sub-ms test now asserts an exact 1_000 ns delta instead of bounded drift, and a regression test pins a size of 10^16+1 (> Number.MAX_SAFE_INTEGER).
Successful file reads record the file’s version and observation time.
Each task maintains independent file observations, with options to check, retrieve, clear, or count them.
Observation tracking is supported for both current and legacy file-reading formats.
Re-reading a file updates its recorded version and observation time.
Bug Fixes
File reads still succeed when version information is unavailable; any existing observation for that file is cleared.
Each Task now owns an ObservationRegistry. Native and legacy text reads record version tokens for resolved paths when available. Token computation failures do not fail successful reads.
Adds FileObservation and ObservationRegistry. Each Task owns a readonly registry. Tests cover timestamps, replacement, lookup, clearing, size, forgetting paths, and registry independence, including parent and child tasks.
Native and legacy text reads record version tokens for resolved paths. If token computation fails or returns no token, the read removes any prior observation without failing. Tests cover successful reads, failed reads, token lookup failures, and registry independence. The suppression count for the read-file tests decreases by one.

No actionable merge-blocking issue remains in the reviewed change; it is ready to merge after normal checks.
Security architecture risk: Low · up to 0cb75
The change does not enable new file-write authority. However, a file changing between the content read and version lookup can produce an observation that does not describe the returned content, limiting its suitability for later write-safety checks.
Retained concerns
Low · security · inferred: The new observation producer can pair returned content with a later file version or replacement identity. Native and legacy reads obtain bytes first, then compute a token through an independent pathname stat. A process already able to modify or replace that file can intervene between those operations. These records therefore do not establish the read-version binding expected by the planned write-safety control. Observations are currently recording-only, so this is not a demonstrated bypass of an active write guard.
Security review details
Security Blast Radius
inferred — The demonstrated integrity issue is bounded to observations for successfully read text paths in an affected Task. Triggering replacement requires existing ability to modify the relevant filesystem path. Separate registry instances prevent this state from automatically propagating into parent or subtask observations; no cross-service privilege expansion is established.
Security Findings and Attack Paths
inferred — A filesystem mutator can replace or change a file after its bytes are read but before the subsequent pathname stat. The observation can then describe the later file while the result contains earlier content. The introduced effect is inaccurate read-version evidence, not a demonstrated unauthorized write: the registry documents future consumption, and inspected production references contain only ownership and observe/forget calls.
Trust Boundaries and Controls
observed — Agent-supplied native arguments reach ReadFileTool through the existing tool handler. Ignore validation and the read-approval flow precede filesystem access and observation updates; the added registry does not itself perform filesystem operations or authorize access.
Resilience and Maintainability Implications
observed — The inspected presenter rejects entry after task abort, uses a per-task presentation lock, and awaits the read tool before releasing that lock. This counters overlapping normal presenter executions, but does not bind observations to returned bytes or establish cancellation of an already-running token lookup.
Hardening Proposals
proposed — Before making observations authoritative for guarded writes, define a stable content-and-identity observation protocol that detects modification or replacement during the read. Treat an unstable observation as unavailable rather than fresh, and explicitly define interruption and recovery semantics for observation ownership.

Explanation
The description is detailed and covers the linked issue, implementation, design intent, tests, stacking context, and known test limitations. It does not use the exact template headings and omits the completed checklist, explicit documentation-impact statement, and contact details, but it is mostly complete and directly relevant.
Full details: Security Boundaries
Explanation
Changed code does not match any failure condition. ReadFileTool still requires rooIgnoreController.validateAccess and user approval before reading (ReadFileTool.ts:148-168, 434-541; legacy path:692-729). The new code only computes a local fs.stat version token, records it in a per-task in-memory Map, or removes a stale entry (ReadFileTool.ts:219-228, 778-814; observationRegistry.ts:20-54). It does not output the token, execute input, or bypass an approval or allowlist control.
Full details: Persistence Integrity
Explanation
No changed persistence path exists. The PR adds an in-memory ObservationRegistry backed by a Map and records observations during reads; it performs no file/database writes and is not serialized through task persistence. The changed read paths await version-token computation and file reads before recording the in-memory observation. Therefore the stated persistence-integrity failure conditions are not introduced.
Full details: Regression Evidence
Explanation
The native text-read path has focused coverage for the changed pre-read token ordering in src/core/tools/__tests__/readFileTool.spec.ts (the readStarted test asserts the pre-read token). The legacy text-read path has the same changed ordering in src/core/tools/ReadFileTool.ts, but its tests use static stat/read mocks. Moving computeVersionToken after fs.readFile in the legacy path would pass all current legacy tests. This is a concrete stale-observation regression scenario.
Resolution
Add a legacy-path ordering test that changes the mocked token during fs.readFile, then assert that the registry stores the token captured before the read. Keep the existing legacy success and token-failure tests.
Full details: Lifecycle Resource Cleanup
Explanation
The new per-task ObservationRegistry retains one entry for every unique file read, but Task.disposeOnce() never clears it. A concrete path is parent-task delegation: delegateParentAndOpenChild() disposes/removes the parent, then constructs the child with parentTask: parent; the child keeps the disposed parent reachable. Any paths and version strings in parent.observationRegistry.entries therefore remain in memory for the child lifetime. This resource was introduced by Task.observationRegistry and ObservationRegistry.entries.
Resolution
Add lifecycle cleanup for the registry in Task.disposeOnce(), such as this.observationRegistry.clear(). Prevent an in-flight read from repopulating a disposed registry, for example by making the registry closed on disposal and ignoring later observe() calls. Add a disposal test that observes entries, disposes a task, and verifies that no entries remain or can be added.

Bind each observed token to the returned file content.
fs.readFile() completes before computeVersionToken() runs. If another process changes the file in that interval, the registry stores the newer token for older returned content. A later guarded write can then overwrite that unseen change.

…lvable
Both token-failure tests started from an empty registry, so deleting the forget() that runs when computeVersionToken fails left them green: nothing was there to drop. Each path now gets a warm-state companion that first performs a successful read (which records the observation for the resolved path, captured through the observe spy) and then performs a second read whose token lookup fails.
Those tests assert the read still succeeds (content preserved, no failure flag), that nothing new was observed, and that the seeded entry is gone via both reg.get(observedPath) and the registry size.
Negative controls, each run against production code and restored byte-identically (sha256-12 ea2c08484c2d):
native forget() removed: AssertionError: expected { ... } to be undefined (warm native test)
legacy forget() removed: AssertionError: expected { ... } to be undefined (warm legacy test)
Both restored and re-ran green; the original cold-start tests are untouched, so each branch keeps cold and warm coverage.
readFileTool.spec.ts + Task.spec.ts after the change: 287 passed. tsc --noEmit clean, eslint --max-warnings=0 --no-warn-ignored clean, suppression counts unmoved, prettier no-op.
…btask
The registry isolation test only showed two standalone registries behaving independently, which says nothing about the parent/subtask contract the write guard is used in, where a subtask is constructed with parentTask and rootTask.
Task.spec.ts now builds a parent and a child through the constructor (startTask: false, no API traffic) and asserts that the child points at the parent, carries the parent's taskId as both parentTaskId and rootTaskId, owns a different registry object, holds its own token for the same path, and that forgetting the child's entry leaves the parent's intact.
One factual note recorded in the test: Task accepts a rootTask option and derives rootTaskId from it, but never assigns the rootTask object field - ClineProvider.ts:3459 reads that field and ClineProvider.spec.ts:1931 patches it by hand - so the test asserts the id link the constructor actually establishes rather than pretending the object link exists.
Negative control (registry shared between instances through a process-level singleton): AssertionError: expected ObservationRegistry{ entries: Map{} } not to be ObservationRegistry{ entries: Map{} } at the identity assertion; Task.ts restored byte-identically (sha256-12 e163f866d049) and re-ran green.
The workflow uploads generated reports in the changed-code-mutation-report artifact. A direct artifact link appears below after upload.
Advisory findings
src/core/task/Task.ts:4370: mutation exclusions must use "// Stryker disable next-line MutatorName: concrete reason"; broad or unreasoned exclusions are not allowed.
Brings in the main-side delta since our base 4f1dee1 (up to e943530). Content merges clean: git merge-tree --write-tree predicted tree 4b7828c with no conflict output, so our unit touches no path main modified in that window.
…review-raised cases
Both read paths computed the observation token after reading the file and after
awaiting trackFileContext, so a mutation in that window left the registry holding a
token newer than the bytes this read returned and the write guard would accept a stale
write. The token is now taken before the read and recorded only after the read
succeeds, on the native and the legacy path alike, through one file-local helper so the
two paths cannot drift apart - the two blocks were character-identical once comments
are stripped, which I diffed line by line before folding them together. The direction
is a deliberate choice: a mutation during the read now yields a token that mismatches
and blocks the guarded write, which is the conservative answer for a guard.
The pinning test mutates the file inside the read so the pre-read and post-read tokens
differ, and names which one the tool may record; with the stat moved back after the
read it reddens - expected '1:2:300:4000000000:5000000000', received
'1:2:300:9000000000:5000000000' - and the implementation is restored from a saved copy
(hash 7053a7861143cb42dd9a0dc0bd3f1a83115d0007 before and after the control).
Also strengthens the assertions review flagged, with no behaviour change: observe
assertions compare the resolved full path instead of a substring, so a call made with
the relative path can no longer pass (native site and both legacy sites); the
beforeEach stat mock now returns the BigIntStats shape the tool reads, because the
partial double made computeVersionToken throw a TypeError that its .catch swallowed,
laundering a mock gap into "no token"; the stale-entry test arms the stat failure on
the call that passes { bigint: true }, the only call that does, instead of on call
parity; and the registry test pins the exact observedAt under a fake clock instead of
accepting any number, which both 0 and NaN satisfied.
Measured: readFileTool.spec.ts + observationRegistry.spec.ts 102 passed, Task.spec.ts
201 passed, check-types 11/11 exit 0, root lint exit 0, eslint clean on the touched
files, and test:solheim-smoke-ci fails identically with these files reverted to the
merge commit, so its red is pre-existing on this merge and not caused here.

Summary
S2 of the file-write safety series (plan: easonLiangWorldedtech/Zoo-Code#33), part of epic #1375. Stacked on S1 (#1383, version token). Introduces the per-task file observation registry (A2): when the agent reads an existing file, the on-disk version token is recorded against the task. The S4 guarded-write will later compare the recorded observation with the token recomputed before a write to detect "the file changed since the read" (stale) or "the file was replaced" (identity change). This PR records observations only — it does not consult them, so behavior is unchanged.

Pure in-memory, zero I/O, no dependencies.src/core/task/Task.ts: each Task owns anobservationRegistryinstance — parent and subtask observations are independent by construction.src/core/tools/ReadFileTool.ts: after a successful read of an existing file, recordscomputeVersionToken(absolutePath)(S1) into the task's registry. A stat failure never fails the read — the token is best-effort (.catch(() => undefined)).Tests
Notes
fs.statper successful read of an existing file — the same call the S4 write guard will re-run, now cached per task.mainand its diff includes the S1 commits. Merge only after feat(file-safety): file version token for the guarded-write path (A1, #1375) #1383 lands (then this becomes a fast-forward).Review-gate re-trigger (2026-08-30): empty commit a00eef8 (no code change) re-runs CI and CodeRabbit current-head review under the org new PR review gate; the code head remains 2965ad1.
Review feedback (2026-09-14 CodeRabbit cycle): 6b821b2 strengthens both version-token-failure regression tests to assert the read content, not just the path (native: "content"; legacy: "legacy content" alongside the existing path checks).
Notes on the current head (dd060a4)
Why the merge of main needed no conflict resolution. This head merges upstream/main
e943530c8, a 1138-file delta over our base4f1dee18e. Only four paths intersect that delta:src/core/task/Task.ts,src/core/task/__tests__/Task.spec.ts,src/core/tools/ReadFileTool.ts, andsrc/core/tools/__tests__/readFileTool.spec.ts. Every other file this unit touches is a path main has not modified in that window, which is why the content merge is clean rather than merely resolved -git merge-tree --write-treepredicted tree4b7828c03with no conflict output before the merge was run.Observation keys are absolute resolved paths, and that is a Windows-shaped detail. The tool records the file it read under the resolved absolute path, not the relative spelling the caller passed. On Windows the resolved key carries backslashes and drive-relative components, so an assertion that the recorded path merely contains
existing.tsis satisfied by the relative spelling too - it cannot tell the two spellings apart on this platform. The observe assertions now compare againstpath.resolve("/test/workspace", "existing.ts")(and thelegacy.tsequivalents), which is the form that actually distinguishes a correctly keyed entry from a mis-keyed one. This is also why the case reads Windows-specific to a POSIX reader: on a POSIX path the two spellings differ more visibly, so the weaker assertion hides less.A red on this head that predates the current work.
pnpm test:solheim-smoke-cifails locally on this merged tree. It was tested by reverting the three files this round touched (src/core/tools/ReadFileTool.ts,src/core/tools/__tests__/readFileTool.spec.ts,src/core/task/__tests__/observationRegistry.spec.ts) back to the merge commit's own content and re-running: the same two assertions fail in exactly the same way with none of the current changes present, so that failure belongs to the merge state and not to this round's edits. Everything this round touched measures green locally:readFileTool.spec.tsandobservationRegistry.spec.ts102 passed,Task.spec.ts201 passed,check-typesexit 0 (11/11 in the pre-push task graph), rootlintexit 0, and the lint-suppressions counts on the touched files are unchanged from the merge baseline. The version-token ordering fix is pinned by a test that mutates the file inside the read itself: with the stat moved back after the read it reddens (expected1:2:300:4000000000:5000000000, received1:2:300:9000000000:5000000000), and the implementation was restored from a saved copy rather than a checkout.
