feat(tools): onParameterParseFailure teardown boundary (split 4/6 of #1066) - #1931
Part of the upstream PR 1066 split. Own issue: 1936. Content source of record: 72143527fd33306e5541116093c2cbf803cce9e0..pr-1066-audit).
Why this unit exists: BaseTool.onParameterParseFailure() teardown boundary plus the WriteToFileTool override, so a truncated final block still finalizes the open partial ask.
Boundaries
base: 4b23b6a27
head: 662691423 (unit content tagged at e3c10401f; the eleven commits after it are the review-driven cleanup and the chain cleanup ports described under Cleanup added after the tag)
Result: PASS — standalone 253 a+d / 3 files (UNDER-SOFT), measured at the tagged head e3c10401f. The head has moved since; see Cleanup added after the tag for what was added on top and how each addition is pinned.

issue: the split plan
Chain position
Merge order is fixed: U12 (1927) -> U4 -> U5 -> U3 -> U6 -> U7 -> FINAL (1928). This PR is opened against main because the split branches live on the fork; the diff GitHub shows is therefore cumulative through this unit. The unit's own content is the delta from the previous unit head (U5), listed under Fidelity above. The sole merge target of the series is the FINAL integration PR (1928); merging the chain in order keeps every bot-visible diff clean.
Cleanup added after the tag (e3c10401f -> 7c11c26ae)
GitHub's diff for this PR at 662691423 is 9 files, +1863 / -32 (cumulative through the chain, as noted under Chain position). On top of the tagged head the unit carries 11 commits, 3 files, +457 / -19, all in WriteToFileTool.ts and its two specs:
rollback-hazard reporting from the failed-partial-stream cleanup, teardown scoped to one task, the failed retry's ask finalized, a completed teardown not reported when the diff revert fails, and the stream state released on execute()'s early returns;
isPartialStreamStillLive() plus a cancellation check after each of the four provider awaits in handlePartial() (provider.getState(), fileExistsAtPath(), task.ask(), diffViewProvider.open()) — the same fix as 1929 2f356e6f7 and 1928 876a93b22. A TaskAborted during any of those awaits tears the state down, and the delta already in flight used to re-ask, re-open a diff view, or stream a partial delta into a view the teardown had released.
The same helper checked at the top of handlePartial()'s catch (662691423): a cancellation that lands while open()/update() is in flight runs the TaskAborted teardown and can reject the call in flight, and the catch then marked the already-released state failed, finalized the ask and ran the failed-stream cleanup a second time — a fresh ask row plus a second rollback for a cancelled task. It now logs and returns, leaving the teardown to own the outcome. Same fix as 1928 3a0065091.
Each addition is pinned by its own negative control rather than a re-measured coverage number: removing one liveness guard -> exactly 1 failed (five separate runs); restored -> green.
changed-line coverage: 20 covered / 0 uncovered — PASS, measured at the tagged head e3c10401f; not re-measured at 662691423, the additions above are pinned by negative controls instead
ESLint: clean on every touched file; suppression counts unchanged
No .changeset file, no CHANGELOG edit.
Recreate policy
If the bot stalls on a pre-merge check and the existing head cannot obtain bot review/approval (empty-commit re-trigger attempted and failed), the unit is recreated from the tagged content source of record — never from a per-PR head. At most 1 PR per issue.

Closes#1936 (unit U3 of the upstream PR 1066 split).
Round update — Lifecycle Resource Cleanup
Shared root cause behind the Lifecycle Resource Cleanup row (all five units of 1066).handlePartial() registers this task's partial-stream entry — and its TaskAborted listener — before it checks the prevent-focus-disruption experiment. With the experiment enabled the delta returns without ever showing a preview and never reaches execute()'s teardown, so the entry and the listener stay attached for the rest of the task's life, and a streamFailed mark armed by an earlier failed delta keeps suppressing this task's later diff previews. The sibling units carry the same release in their own PRs, each verified red-first with a negative control.
This unit (U3): the execute() exits already tear down explicitly (the validation returns, both approval denials, success and the catch each call super.resetPartialState() + resetTaskPartialState(task)), so only the suppressed-preview return in handlePartial() was missing; it now releases too.
Red first: the new test failed with expected 1 to be +0. Green: 49 passed / 5 skipped. Negative control: removing the release turns exactly that one test red; restored green.
Main refresh. Merged org main 036245c5e (U1 1927). U1's content no longer appears in this diff: 9 files +1863/−32 → 6 files +1416/−27, 0 behind main. Conflicts were confined to src/core/task/__tests__/Task.spec.ts (and Task.ts on U7) — the region U1 rewrote; resolved by taking main's version of the shared save-stage tests (try/finally plus the fixed-task-id ui_messages.json cleanup from cf9206a42) rather than re-implementing U1.
Verification after the merge: Task.spec 172 passed, writeToFileTool.spec 49/5, presentAssistantMessage-custom-tool 20, DiffViewProvider.spec 71, eslint 0/0.
Rows from the 2026-10-09 review
The scope extension needed by the Persistence Integrity row was issued in the split plan before the fix was pushed.
Persistence Integrity - diff-open is now transactional: the catch awaits revertDiffChangesBeforeReset() before resetting, and DiffViewProvider.revertChanges() (ported verbatim from 1930 7b783452c / 6cae369d9) removes the placeholder and only the directories this operation created, even when open() failed before an editor existed.
Lifecycle - execute()'s guarded scope starts before the preflight filesystem work and ends with an unconditional finally { resetTaskPartialState(task) } (same shape accepted on 1930).
Regression Evidence - focused execute() tests seeding the task's stream state first: missing content, successful completion, a failing write, the preflight throw, and the open()-failure rollback.
Fix commit: 41ccb425b. Negative controls: neutering the finally -> exactly the five exits it owns go red; moving the try back -> exactly the preflight test red; removing the revert call -> exactly the open-debris test red.
CodeRabbit rows at head 1b09bab32 (fixed in 2a9bfabfb)
Major - saveChanges() persists the document and then keeps working (closing the diff
views, tab bookkeeping, diagnostics), and trackFileContext() / pushToolWriteResult() run
after it. A failure past that point rolled the tool call back and restored the previous
content, or unlinked a file the user had just approved being created. saveChanges() now
signals its commit point at the document save through an optional callback, and the rollback
stands down from there; the saveDirectly() branch marks the same point when it returns,
since performing the write is what it does (its signature is untouched).
Minor - the prevent-focus-disruption rejection exit returned without resetting the
provider, leaving this call's adopted created directories and editType for the next write's open() to fold into its own rollback.
Minor - the catch called diffViewProvider.reset() directly, so a rejecting reset skipped reportRevertFailure(); it now uses the resilient helper the other two teardowns use.
Same invariant as the landed rule in Follow-up 11 (6092123982), delivered in 1921:
the commit point is where a write becomes irreversible, not where the call finished.
Four new tests; each is the only failure when its own guard is removed. 5ff7bea73 is
formatting only: pnpm format:check became a required step of the compile job on main in 667ff58 and runs on the merge commit, and these four files - all inside this unit's own diff
were already flagged at 1b09bab32. No file outside the diff was reformatted.
Split-unit issue reference
No approved upstream issue exists for this change. This pull request is one unit of a declared split of a larger change, and the split plan, the unit boundaries and this unit's acceptance criteria are recorded on the tracking issue in this repository: #1989. That issue is the home for this unit's review dispositions and follow-up registrations, which is why no approved issue is linked above.
Improved recovery from invalid file-writing requests and filesystem errors, including cleanup of partial edits in the diff view.
Canceled or failed file-writing streams stop updating the diff view; partial changes are reverted where possible.
Failed edits to new files clean up placeholder files and created folders when safe, preserving content if restoration or tab closure fails.
Failed document saves are treated as errors, and edits are reverted where possible.
Improved cleanup after failed task-history restoration to prevent stale file-editing state from affecting later work.
WriteToFileTool now manages partial-stream state and failures per task. Parse failures and stream failures trigger cleanup and diff rollback. DiffViewProvider tracks commit state and checks editor and filesystem rollback results.
BaseTool finalizes a partial tool ask and calls onParameterParseFailure. WriteToFileTool rolls back the diff view and clears task state. When a streaming error was retained, it reports that error instead of the parse error. Tests cover teardown and failure reporting.
Per-task streaming and execution lifecycle src/core/tools/WriteToFileTool.ts, src/core/assistant-message/presentAssistantMessage.ts, src/core/webview/ClineProvider.ts, src/core/tools/__tests__/writeToFileTool.spec.ts, src/__tests__/removeClineFromStack-delegation.spec.ts
Partial streaming checks task-state liveness and does not create parent directories. Execution and stream failures finalize partial asks, revert and reset the diff view, and clear task-specific state. Assistant-message and provider cleanup also clear task state. Tests cover cancellation, task isolation, and cleanup order.
Diff-view commit and rollback src/integrations/editor/DiffViewProvider.ts, src/integrations/editor/__tests__/DiffViewProvider.spec.ts, src/utils/fs.ts
DiffViewProvider checks save outcomes and tracks directories created outside the provider. Rollback restores file buffers and removes placeholders and directories, tolerating ENOENT. Failed or refused tab closure stops filesystem cleanup. Directory creation also removes directories created earlier in the same call when a later creation fails.

Explanation
The description explains the purpose, implementation, scope, and reported tests, and it identifies issue #1936. It does not use several template sections, including the checklist and documentation impact section. It also conflicts with itself by saying the PR closes #1936 and later saying no approved issue is linked.
Full details: Linked Issues check
Explanation
[#1936] Objective: add a BaseTool.onParameterParseFailure() teardown boundary and a WriteToFileTool override. At head 16a7b15, src/core/tools/BaseTool.ts defines protected onParameterParseFailure(task, callbacks, parseError): Promise<boolean>, defaulting to false. handle() calls task.finalizePartialToolAsk() and then this hook on the native-argument parse-failure path, before execute() would run. The generic parse error is reported only when the hook returns false. src/core/tools/WriteToFileTool.ts overrides the hook. It looks up the per-task stream state, reverts the diff document (revertDiffChangesBeforeReset) before tearing down the state, resets the diff view, and reports a rollback failure to the user when the revert fails. When a streaming error was retained, it reports that error through handleError("writing file", ...) and returns true, which suppresses the parse error. Test files (writeToFileTool.spec.ts, writeToFileTool-partial-state-cleanup.spec.ts) are reported to cover parse-failure teardown, diff rollback, state release, streaming-error precedence, cancellation, and early exits. I did not read those test files in this review. The reported test pass counts and the tagged-head changed-line coverage are reported outcomes, not verified here. Coverage was not re-measured at the current head, and that gap is noted but is not a coding requirement failure.
Full details: Out of Scope Changes check
Explanation
The changes outside the three named unit files support [#1936]. The boundary path calls task.diffViewProvider.revertChanges() to restore the diff document, as the objective requires. DiffViewProvider.ts therefore provides a method the in-scope override depends on. The fs.ts change (createDirectoriesForFile removes directories it created before a later mkdir failure) supports the same rollback. ClineProvider.ts and presentAssistantMessage.ts clear the same task-local write state on disposal and missing-nativeArgs paths. The PR objectives state that the author considered DiffViewProvider work to belong to #1916. That is a split-plan disposition. It does not change the code connection shown above, so I did not treat the DiffViewProvider changes as unrelated. I did not read the full WriteToFileTool.ts or the whole-PR diff. The incremental diff against the previous reviewed head was unavailable (changed blobs missing from checkout), so this verdict relies on the whole-PR summary and the file read at head. No demonstrated unrelated change appears in the reviewed scope.
Full details: Security Boundaries
Explanation
A changed malformed-argument path bypasses diff rollback. presentAssistantMessage.ts:601-605 adds clearTaskState() and finalizes the partial ask when write_to_file has no nativeArgs, but it does not call revertChanges(). The normal WriteToFileTool.onParameterParseFailure() rollback at WriteToFileTool.ts:249-275 is not reached because this branch exits before BaseTool.handle(). A partial stream can open and update a dirty diff document at WriteToFileTool.ts:636-653 before execute() reaches its approval gate at lines 467-468. DiffViewProvider.reset() only closes and clears state; it does not restore document contents. If final arguments are malformed, the user can save the partial unapproved content.
Resolution
Route the missing-nativeArgswrite_to_file branch through a shared parse-failure teardown. Revert the diff before clearing task state, reset the diff provider afterward, and report rollback failures without claiming teardown succeeded. Add an integration test that streams a partial write, supplies malformed final arguments, verifies rollback occurs before reset, and confirms the unapproved content cannot be saved.
Full details: Persistence Integrity
Explanation
The missing-nativeArgs path still skips rollback. handlePartial() writes streamed content into the diff document (WriteToFileTool.ts:650-653). When the completed block lacks nativeArgs, presentAssistantMessage.ts:601-605 clears task state and finalizes the ask, but does not call onParameterParseFailure(), revertChanges(), or reset(). The new WriteToFileTool.onParameterParseFailure() does perform revert-before-reset (WriteToFileTool.ts:249-270), but this path bypasses it. A malformed final block after a partial write can therefore leave unapproved content in the dirty editor document, where a user can save it. DiffViewProvider.reset() closes views and clears provider state; it does not restore the document.
Resolution
Route the missing-nativeArgs branch through the write-to-file parse-failure teardown, or expose an equivalent public teardown method. Await revertChanges() before reset(), report rollback failure, finalize the partial ask, and release per-task state. Add a regression test that streams a partial write, supplies a completed block without nativeArgs, and verifies rollback occurs before reset and the dirty content cannot be saved.
Full details: Regression Evidence
Explanation
The changed execute() commit guard lacks focused coverage for the prevent-focus-disruption path. WriteToFileTool.ts:438–441 sets writeCommitted after saveDirectly() resolves, and :485–491 can then fail in trackFileContext() or pushToolWriteResult(); the catch skips rollback when committed. The tests cover a saveChanges() commit failure at writeToFileTool.spec.ts:1215–1231, but the prevent-focus test at :1187–1213 rejects approval and verifies that saveDirectly() is not called. No focused test approves this path, lets saveDirectly() succeed, and triggers a later failure.
Resolution
Add a focused WriteToFileTool.execute() test with prevent-focus-disruption enabled. Approve the write, make saveDirectly() resolve, then reject trackFileContext() or pushToolWriteResult(). Assert that handleError() and reset() run and revertChanges() does not run. Ensure the test fails if the writeCommitted = true assignment or the commit-aware catch guard is removed.
Full details: Lifecycle Resource Cleanup
Explanation
The pending-open lifecycle leak remains. WriteToFileTool.handlePartial() awaits diffViewProvider.open() and checks stream liveness only after it resolves (WriteToFileTool.ts:638-648). On abort, Task.disposeOnce() calls revertChanges() while the provider is editing (Task.ts:3717-3721). The changed rollback path can reset the provider before the pending open completes (DiffViewProvider.ts:606-613, 648-667, 741-747). The open continuation then assigns activeDiffEditor and registers three listeners after its await (DiffViewProvider.ts:222-223, 237-295); the tool's liveness return does not clean them up. A cancellation during openDiffEditor() can therefore leave a diff view and listeners attached after disposal. The new tests mock open() and do not exercise a pending real editor open followed by abort and late resolution.
Resolution
Coordinate cancellation with DiffViewProvider.open(). Invalidate pending opens when revertChanges() or reset() starts. Check the open generation after every await and before assigning activeDiffEditor or registering listeners. Dispose the pending openDiffEditor() timeout and event subscriptions on cancellation. Serialize pending open() with rollback/reset, or make a late continuation close the editor and dispose all resources. Add a regression test that aborts while the real openDiffEditor() is pending, resolves it afterward, and verifies that no diff view, active editor, or provider listeners remain.

…partial-path case
Same flaky case as the u5 run: the core project passes locally at this head and
the case passes in isolation. Re-running to confirm.
… no-filesystem contract
Same fix as p1066/u5 (6906c02): platform-unit-test (ubuntu-latest) fails here and passes locally because
the failing case is it.skipIf(process.platform === "win32").
The case asserted that the second streaming delta calls createDirectoriesForFile - the exact call this
chain removes, because an unguarded mkdir in handlePartial threw EROFS into BaseTool.handle() without
setting didRejectTool/didAlreadyUseTool, stalling the agent loop. The chain's own regression test pins the
new contract; this older case still pinned the old one, so the two contradicted and only Linux CI saw it.
Rewritten as "defers parent directory creation to execute() while streaming": no filesystem work while
streaming, directories still created by the authoritative non-partial execute().
Local run: the file passes; the rewritten case also passes with the win32 skip lifted.

…inalize the failed retry's ask
Two findings on this unit's own state:
1) resetPartialState() is overridden to clear the whole taskPartialStreamState map, and execute() calls it
on both the success and the error path. The map is keyed per task precisely so two providers can stream
write_to_file through this singleton at once, so task A's execute() was deleting task B's entry while B
was still streaming: B loses streamFailed (its next delta re-opens the diff view and spawns a duplicate
partial ask - the exact case the stabilization guard prevents) and loses streamError.
execute() now calls super.resetPartialState() (the base field is genuinely instance-global) plus
resetTaskPartialState(task) for this task only.
2) On the diff-view branch execute() opens its own partial ask before the write. If the write then throws,
the catch reported the error and reset without finalizing that ask, leaving the spinner and Save/Reject
buttons live for a tool call that had already failed. The catch now finalizes the pending ask first.
Tests (writeToFileTool.spec.ts, new per-task stream state isolation block): a second streaming task keeps
its streamFailed/streamError across another task's execute(), and a failing save finalizes the ask with the
exact partial payload. Both fail on the pre-fix code (2 failed / 38 passed) and pass after (40 passed).
Local: eslint clean on both files with --prune-suppressions (no suppression change), package tsc clean.

2a9bfabfb - commit-point tracking so a failure after the write lands can no longer roll

cannot swallow the rollback report.

saveChanges() awaited the document save and dropped the returned boolean, so a
save that reported failure still ran the success bookkeeping and the caller was
told the approved content persisted. Capture the awaited result and throw a
typed DocumentSaveFailedError before the commit point and before any success
bookkeeping, so the caller keeps its rollback and the existing failure handling
runs. A true result behaves exactly as before; a save that rejects still
propagates as before.
This branch gates an on-commit callback after the save rather than a placeholder
release, so the ordering promise is derived for this branch: the callback must
not run when the save reported false. The statement is inherited from upstream
code that predates the chain; this branch is the earliest carrier that is still
open, so the shape is fixed here and the later branches port it forward
re-derived.
The regression doubles resolve an explicit false, so a double resolving
undefined cannot pass by accident. Negative controls: reverting to a bare await
reddens exactly the two save-false tests (2/88, the same two as the pre-fix
production run); moving the throw after the callback reddens exactly the
ordering test (1/88). Restored byte for byte, hash verified.

Learning: In src/core/tools/WriteToFileTool.ts and src/integrations/editor/DiffViewProvider.ts, rollback must stop at the durable write commit point, not when the full save operation returns. DiffViewProvider.saveChanges() signals that point through its optional onCommit callback after a successful document save, or when the document is already clean. Later bookkeeping failures must not restore old content or delete an approved new file; failures before commit must retain rollback.
…t applied
restorePreStreamBuffer() awaited vscode.workspace.applyEdit() and dropped the
returned boolean, so an edit the workspace reported as not applied was treated
as applied: saveBufferClean() then saved a buffer that could still hold the
unapproved streamed content, close() could succeed on the resulting clean tab,
and the rollback reported success for a restore that never happened. Capture
the result the way the close in discardFileTab() already does, treat false as
a failed restore, keep a rejection as the cause, and throw before
saveBufferClean() runs so no save, close, or delete bookkeeping continues on
the assumption that the buffer came back. This is the same defect family as
the discarded document-save result fixed in the previous commit, and the shape
is taken from it.
The regression double resolves an explicit false, so a double resolving
undefined cannot pass by accident; a second test pins the rejection as the
cause, and a third pins the applied-edit path so the success flow is unchanged.
Negative control: reverting the call site to the bare await (the pre-fix
production file, blob 95a038a) reddens exactly
the two failure tests (2/91, the same two as the pre-fix production run) and
leaves the applied-edit guard green. Restored byte for byte, hash verified.
The thread's second paragraph asks for the same check on document.save() in
saveBufferClean(); its stated consequence, a dirty tab whose close is refused,
is already converted into a rollback failure by the close check in
discardFileTab(), and the only caller deletes the file a moment later, so that
half is dispositioned with evidence in the thread rather than changed here.
see review comment 4240879376
…save reports false
revertChanges() awaited the workspace edit and the document save of its
existing-file branch and dropped both returned booleans, so a refused edit or
a refused save was reported to the caller as a completed rollback. The
parse-failure and partial-stream cleanup paths gate their failure report on
revertDiffChangesBeforeReset(), which can only see a throw, so the user was
told the teardown was complete while the document could still hold the
unapproved content, or the restored original content could still be missing
from disk. Validate the edit result before the save runs and the save result
before any later bookkeeping, capture each result the way the pre-stream
restore and the refused close already do, and keep a rejection as the cause.
An applied edit plus a landed save behaves exactly as before; the new-file
branch is untouched, its close check is already pinned.
This is the second call site of the defect family this branch already fixed
twice - the discarded document-save result in saveChanges() and the ignored
restore edit in restorePreStreamBuffer() - and the shape is taken from them.
The site was registered rather than fixed in the previous round; a
registration does not clear a pre-merge row, only code at the head does, so
it is fixed here.
The regression doubles resolve an explicit false, so a double resolving
undefined cannot pass by accident; a test pins each rejection as the cause
and a success guard pins the unchanged applied-and-saved flow. The shared
editor double now reports a save that landed, because the rollback reads the
result. Negative controls: reverting the edit call site to the bare await
reddens exactly the two edit tests (2/96), reverting the save call site
reddens exactly the two save tests (2/96), and the pre-fix production file
reddens exactly the four failure tests (4/96) with the success guard green.
Restored byte for byte, hash verified.

- presentAssistantMessage: torn down write_to_file partial state and finalized its partial ask on the missing-nativeArgs guard path (handle() never runs there)
- WriteToFileTool: run the abort-window rollback/reset in a finally around handleError, which throws once the task is aborted
- DiffViewProvider: discardFileTab also matches TabInputTextDiff so a dirty new-file diff document cannot survive the rollback
- DiffViewProvider: record the commit on the provider and make revertChanges() stand down after it, so abort-driven teardown cannot undo an approved write
BaseTool.onParameterParseFailure is a no-op base hook, so its parameters are underscore-prefixed; the cleanup spec no longer defines an unused stateFor helper.

Restore the diff before clearing write_to_file state on this path.
When a final write_to_file block lacks nativeArgs, this branch skips revertChanges(). Streamed content stays in the open diff document, and a user can save it. Call revertChanges() before clearTaskState(cline), as onParameterParseFailure() does. This path predates the PR, but the new teardown block is where the fix belongs.

Revert the streamed diff before resetting on an empty path.
When the final write_to_file block has an empty path after a partial stream opened a diff, the !relPath return calls diffViewProvider.reset() without reverting. reset() leaves the dirty diff buffer open, so unapproved content stays saveable.

In the empty-path branch of the final write_to_file block, revert any streamed
diff before calling diffViewProvider.reset() so unapproved partial content is
not left saveable. Keep the existing reset behavior for other exits unchanged.
Add a presenter test for the write_to_file missing-nativeArgs teardown.
The branch at presentAssistantMessage.ts:589-597 calls writeToFileTool.clearTaskState(cline) and then cline.finalizePartialToolAsk(). No presenter test in the inspected suites reaches this branch. The direct tests call clearTaskState in isolation, so removing the presenter call would not fail any inspected test.
The call matters after an earlier streaming filesystem failure. That failure leaves a streamFailed entry. While the entry exists, handlePartial returns before it renders the preview of a later write_to_file call in the same task. The current code clears the entry on this path, so this is a coverage gap rather than a current runtime failure.
A regression test should stream a failing write_to_file delta, then send a complete write_to_file block without nativeArgs. It should assert that the stream entry is cleared.
Verification notes
The presenter-level wiring is not reachable from the direct WriteToFileTool tests. Those tests cover clearTaskState itself. The presenter call at line 593 is covered only by the presenter layer. No explicit repository requirement is violated, so this is a refactor-priority improvement.

Cover the commit-callback order when post-commit bookkeeping rejects.
The clean-document test stubs closeAllDiffViews() to resolve and asserts only the call count of onCommit. Moving onCommit?.() below closeAllDiffViews() would still pass this test. The caller stops rolling back once the commit callback runs, so no test detects a reordering that would skip that signal when bookkeeping rejects.

Unverified finding
We couldn't verify this from the code. It is shown for review, not as a verified issue.
An update() already in flight is not synchronized with abort-driven reversion. If its workspace edit lands after revertChanges(), the late update can re-dirty the document despite the stream state having been cleared.
Unapproved partial content can reappear in the editor after cancellation and remain saveable.
This can occur when: The task aborts while vscode.workspace.applyEdit() within a partial update is pending; rollback finishes first and applyEdit then succeeds.

Return the directories that survive a failed cleanup.
If fs.mkdir fails after parent directories were created, the catch block removes them best-effort. If one fs.rmdir also fails, that directory stays on disk. createDirectoriesForFile then throws without returning it, so callers cannot clean it up. The PR narrows this existing gap but does not add it. A safe fix needs a change to the helper's error contract, so no edit is proposed here.

Keep the placeholder check from silently skipping cleanup on stat errors.
When fs.stat fails with a non-ENOENT error, the editorless branch of revertChanges() skips removeCreatedFile(). The placeholder created by open() stays on disk, and reset() then clears editType. The next write's fileExistsAtPath() check sees the placeholder and selects the modify path, so the write can treat the new file as an existing one.

Stat-then-unlink in the editorless rollback can delete content written after the size check.
655-660: Minor · Data Integrity & Integration · Poor tradeoff
Stat-then-unlink in the editorless rollback can delete content written after the size check.
The editorless branch at src/integrations/editor/DiffViewProvider.ts:655-660 calls fs.stat, checks stats.size === 0, and then calls removeCreatedFile. That helper calls fs.unlink at lines 585-593. The stat and the unlink are separate awaits. The size check covers only the content present when stat resolves. If another process writes to the placeholder path after stat and before unlink, the rollback deletes that file. This applies to both empty and non-empty content. The check does not preserve non-empty content written in this window.
Path-based stat and unlink do not form an atomic check-and-remove, and the inspected code has no safe local narrowing for this window.

The new test at lines 979-991 reads writeCommitted only after saveChanges() resolves. On top of the tagged head the unit carries 11 commits, 3 files, +457 / -19, all inWriteToFileTool.tsand its two specs:execute()'s early returns;isPartialStreamStillLive()plus a cancellation check after each of the four provider awaits inhandlePartial()(provider.getState(),fileExistsAtPath(),task.ask(),diffViewProvider.open()) — the same fix as 19292f356e6f7and 1928876a93b22. ATaskAbortedduring any of those awaits tears the state down, and the delta already in flight used to re-ask, re-open a diff view, or stream a partial delta into a view the teardown had released.handlePartial()'s catch (662691423): a cancellation that lands whileopen()/update()is in flight runs theTaskAbortedteardown and can reject the call in flight, and the catch then marked the already-released state failed, finalized the ask and ran the failed-stream cleanup a second time — a fresh ask row plus a second rollback for a cancelled task. It now logs and returns, leaving the teardown to own the outcome. Same fix as 19283a0065091.Each addition is pinned by its own negative control rather than a re-measured coverage number: removing one liveness guard -> exactly 1 failed (five separate runs); restored -> green.

If the bot stalls on a pre-merge check and the existing head cannot obtain bot review/approval (empty-commit re-trigger attempted and failed), the unit is recreated from the tagged content source of record — never from a per-PR head. At most 1 PR per issue.

Round update — Lifecycle Resource Cleanup
Shared root cause behind the
Lifecycle Resource Cleanuprow (all five units of 1066).handlePartial()registers this task's partial-stream entry — and itsTaskAbortedlistener — before it checks the prevent-focus-disruption experiment. With the experiment enabled the delta returns without ever showing a preview and never reachesexecute()'s teardown, so the entry and the listener stay attached for the rest of the task's life, and astreamFailedmark armed by an earlier failed delta keeps suppressing this task's later diff previews. The sibling units carry the same release in their own PRs, each verified red-first with a negative control. This unit (U3): the
execute()exits already tear down explicitly (the validation returns, both approval denials, success and the catch each callsuper.resetPartialState()+resetTaskPartialState(task)), so only the suppressed-preview return inhandlePartial()was missing; it now releases too.Red first: the new test failed with
expected 1 to be +0. Green: 49 passed / 5 skipped. Negative control: removing the release turns exactly that one test red; restored green.Main refresh. Merged org main
036245c5e(U1 1927). U1's content no longer appears in this diff: 9 files +1863/−32 → 6 files +1416/−27, 0 behind main. Conflicts were confined tosrc/core/task/__tests__/Task.spec.ts(andTask.tson U7) — the region U1 rewrote; resolved by taking main's version of the shared save-stage tests (try/finally plus the fixed-task-idui_messages.jsoncleanup fromcf9206a42) rather than re-implementing U1.Verification after the merge: Task.spec 172 passed, writeToFileTool.spec 49/5, presentAssistantMessage-custom-tool 20, DiffViewProvider.spec 71, eslint 0/0.
