Moves the classes extracted from FetcherBolt, plus FetchTimeoutHelpers and
ProtocolMetrics, to org.apache.stormcrawler.fetcher, so that o.a.s.bolt only
holds bolts. They become public and say in their Javadoc that they are for
internal use only; the fetch queues expose accessors rather than their maps
and atomics. ProtocolMetrics now logs under its new package.
