feat(web): set security headers on every web response - #1068
This patch mitigates web vulnerabilities including XSS attacks, clickjacking, and insecure connections by integrating Helmet middleware to set security-related HTTP headers such as HSTS, CSP, and X-Frame-Options. Note: the helmet package was not installed because the package manager failed to execute; manual installation via pnpm install helmet is required.

Summary by CodeRabbit
Security
Web app responses now include standard security headers, including strict transport security, content-type protection, and same-origin framing restrictions. Existing content security restrictions remain in place, and the dashboard’s own Grafana frame continues to work.
createWebApp now applies Helmet security headers. Helmet’s default Content-Security-Policy is disabled. Tests check selected security headers and verify that the response has no Content-Security-Policy header.
Adds Helmet as a production dependency and configures middleware with its default Content-Security-Policy disabled. createWebApp applies the middleware. Tests check selected headers, the absence of Content-Security-Policy, and a 200 response. The changeset describes the headers and CSP behavior.
cameri/nostream#1070: The issue describes the same Helmet dependency, CSP compatibility, header tests, and changeset covered by this pull request.
Merge Risk: Low · up to d6b98
The change is mergeable with a bounded test-coverage gap: an automated app-level check would help catch future header or CSP integration regressions.
Security architecture risk: Low · up to d6b98
The change adds browser security controls while preserving the existing nonce-based Content-Security-Policy. No introduced security bypass was demonstrated. Public HTTPS coverage, cross-origin compatibility, and browser-cached transport policy during rollback remain deployment-dependent.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The changed browser policies apply across ordinary responses from this web application, including static resources, public endpoints, and mounted administrative and API routes. Public proxy behavior, external consumers, and affected domain topology are not established by this registration.
Security Findings and Attack Paths
observed — The supplied security assessment retains no verified findings and defers one candidate for a missing verification receipt. Its cited null-CSP assertion belongs to an isolated Express app containing only the Helmet wrapper. Production separately installs the nonce-based CSP, so that assertion does not demonstrate a production CSP bypass.
Trust Boundaries and Controls
observed — CSP ownership remains with the application: Helmet's policy is disabled, while the existing middleware sets the nonce and allowed resource origins. This preserves the pre-existing control rather than relying on the standalone Helmet wrapper to provide CSP.
Hardening Proposals
proposed — Validate effective headers at the public HTTPS boundary and compatibility with intended cross-origin consumers before rollout. Include browser-cached HSTS recovery in rollback guidance rather than assuming image replacement immediately reverses transport policy.

Helmet's remaining defaults ship as-is; its Content-Security-Policy is
disabled, because the app already emits one carrying the per-request nonce
and the CDN origins the pages load from, and a second CSP would intersect
with it and block the nonce'd inline scripts.
Verified against the composed app, not just the middleware: /favicon.ico
returns 200 with nosniff, SAMEORIGIN, max-age=31536000, referrer-policy and
exactly one CSP — the app's own, with frame-src intact.

[Aikido] AI Fix for Express is not emitting security headers
feat(web): set security headers on every web response
Nov 7, 2026
The finding stands; the patch could not ship as written. I completed it on this branch in d6b98ab.
Why the original patch had to change. It imported helmet without installing it — Aikido's own note says installation failed — so Build check failed on a module that does not exist. It also called app.use(helmet()) with defaults, and that would have broken the dashboard: createWebApp already emits its own Content-Security-Policy, carrying a per-request nonce used by the inline scripts and the CDN origins the pages load from. Helmet's default policy would be sent as a second CSP header, and a browser enforces the intersection of every CSP on a response — the nonce is absent from Helmet's policy, so those scripts would be blocked. The fix keeps one policy, owned by the app.
What landed
helmet@^8.3.0 added to dependencies with the lockfile; securityHeaders() in src/utils/security-headers.ts applies every other Helmet default (HSTS, nosniff, X-Frame-Options, Referrer-Policy, X-DNS-Prefetch-Control, X-Permitted-Cross-Domain-Policies, cross-origin isolation) with contentSecurityPolicy: false.
A unit test on the middleware, plus a real check against the composed app: /favicon.ico returns 200 with x-content-type-options: nosniff, x-frame-options: SAMEORIGIN, strict-transport-security: max-age=31536000; includeSubDomains, referrer-policy: no-referrer, and exactly one content-security-policy — the app's own, with frame-src intact.
A changeset (minor), and a PR title that passes the conventional-commit check.
Two things worth knowing before merge
X-Frame-Options: SAMEORIGIN means the relay's web UI can no longer be framed by another origin. The dashboard's own Grafana frame is outbound and unaffected.
referrer-policy: no-referrer applies to every page the relay serves, links included.

Test the composed app’s security headers.
Add a request test for createWebApp() that checks the Helmet headers and exactly one nonce-based CSP, including frame-src. The isolated fixture cannot detect a registration or CSP composition regression in the app.
