Changes

safe_http_url and safe_wss_url use REPLACE_WITH_TOKEN as the placeholder.
Removing label from quicknode_endpoint clears it.
quicknode_endpoint no longer tracks tokens, http_url_with_token or wss_url_with_token, which went stale when tokens were added or removed.
quicknode_endpoint_token exposes http_url_with_token and wss_url_with_token for its token.
request_filters is removed from the endpoint resource's security_options; the data sources still report it.
New quicknode_endpoint_urls data source, including safe_multichain_urls and multichain_urls_with_token.
Headers on the provider index table.

Breaking changes are listed in CHANGELOG.md under 0.2.0.
