What changed in this PR
Adds failed-only counting to @routup/rate-limit, avoiding quota consumption by successful requests still in flight.
Changes:
- Introduces
countmodes and optional store reads. - Shares quota handling between modes.
- Adds documentation and failure/concurrency tests.

Confirmed. It cannot be fixed inside the middleware, though: the handler event routup 6 passes to a handler does not expose error, so when no error handler answers, next() gives this middleware nothing to classify. This was already true before this PR. It is now documented in the README and pinned by tests both ways: with an error handler, the predicate judges the error's response; without one, the request counts as failed. Exposing the error to upstream handlers is a change for routup itself.

Use String literal of RatelimitCountMode so you can use the enum Reference or String Value

No longer applies: RateLimitCountMode and the count option are gone, since the concurrency finding made them redundant with skipSuccessfulRequest.

Calculate skipped-request quota from the post-decrement count.
When another request increments the same key before this request finishes, totalHits is stale. The decrement updates the shared count, but the handler uses that snapshot to set current and remaining. This can report an extra slot compared with the store.
Make Store.decrement return the post-decrement count atomically, then use it for both values.

When skipSuccessfulRequest or skipFailedRequest takes a request back off the counter, the RateLimit-Remaining header (and the request's rate limit info) now reports the count after the decrement instead of before it, on the returned response as well as on a fallback response. A response with immutable headers is copied. Document that a throw no error handler answered reaches the middleware as no response and is counted as failed, and pin both that and the reserving behaviour that bounds concurrent failed requests by max.

What changed since the first version
The first version added a
count: 'failed'option that read the counter before a request and incremented it only after a failure. The review showed that it does not bound a parallel burst: withmax: 2, ten concurrent failing requests all reached the handler. The fix the review proposed, reserving a slot before forwarding and releasing it on success, is exactly whatskipSuccessfulRequestalready does. With the samemax: 2, that option lets 2 of the 10 through. So the option is dropped, and this PR now only fixes what the review found inskipSuccessfulRequest/skipFailedRequest.Changes
RateLimit-Remainingand the request's rate-limit info now report the count after the decrement. Before, a skipped request advertised one remaining slot fewer than it really had. The header is updated on the returned response and onevent.response, so routup's fallback response is covered too. A response with immutable headers (Response.redirect, a forwardedfetch()) is copied.next()returns no response and the handler event exposes no error. The middleware therefore cannot see the status and counts the request as failed. The README now says so.Tests
5instead of4).Response.redirect(immutable headers).max: 2with ten concurrent failing requests lets exactly 2 reach the handler.10 tests pass; lint and the type build are clean.
