Replace the nested thinking dictionary ({ effort, includeThoughts })
with a top level thinkingEffort option and expectedOutputs:
[{ type: "text", languages: ["en"] }, { type: "thought" }] to match how
Tool Use negotiates structured outputs.
Fixes webmachinelearning#222
