feat(tools): guarded write core under the shared lock (U5, #1375) - #1914
This PR adds guardedWrite, the single publish entry point every write tool will use. It turns "last writer wins silently" into "a write that was not earned by a read fails with a remediation the model can act on": create-if-absent for new files, compare-and-swap on the version token for observed files, a cancellation re-check before publication, and the read-first / re-read-then-retry remediation text on refusal. All publishes for one path run on a per-path FIFO chain, so concurrent writes to the same file are ordered.
It is the fifth of nine PRs for epic #1375 (chain tracked on #1991), and the core the remaining units wire their tools into.
guardedWrite(): resolves the target, checks containment, picks the guard from the task's ObservationRegistry - create-if-absent, recreate, compare-and-swap on the version token, or the unobserved-edit remediation - and runs the publish on the per-path FIFO chain.
createIfAbsent and replaceIfVersion semantics: a stale version token or an unobserved overwrite is refused instead of clobbering the file.
Cancellation is re-checked when the link is dequeued, before publication - a write cancelled while queued does not publish.
The model-facing remediation on rejection: re-read the file, then retry.
Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on main and carries its own copy of the earlier units' content, which the GitHub diff shows as context.
Review focus:src/core/tools/guardedWrite.ts and src/core/tools/__tests__/guardedWrite.spec.ts.
Test Procedure
From the repository root, with dependencies installed:
Environment: Windows 11, Node 20, unit layer only.
Pre-Submission Checklist
Scope is one gate scope and matches the unit.
No .changeset or CHANGELOG changes (AGENTS.md).
src/eslint-suppressions.json unchanged - no suppression count increased.
New code lints clean (--max-warnings=0) rather than relying on suppressions.
Tests added at the lowest layer that would have caught each finding.
Visual Snapshot item considered: not applicable (no rendered surface).
Visual Snapshots
Not applicable - no rendered surface.
Videos (interaction / animation only)
Not applicable.
Documentation Updates
No user-facing documentation change: the remediation text is model-facing and introduced with this unit's guard. No .changeset and no CHANGELOG edit (AGENTS.md).
Additional Notes
Budget: 1277 a+d / 418 changed executable lines. 1277 a+d is above the 1000 cap - documented deviation: guardedWrite.ts is a new file and its spec tests that file as a unit, so the two cannot be separated without breaking the fidelity contract.
mutation-diff is red on the changed-line budget only: the gate counts the GitHub diff, which carries the unlanded predecessors (786 changed executable lines against the 500 cap; this unit's own delta is 418, inside the cap). The row clears when the predecessors land and this branch is rebased.
Two Minor findings fixed with regression tests and negative controls:
Copy-fallback durability: the COPYFILE_EXCL fallback creates a NEW
inode at the target that nothing fsynced (the staged inode's pre-commit
fsync does not cover it; step 4b flushes only the parent directory).
The copied target is now opened and fsynced right after the fallback;
failure reports PostCommitDurabilityError (new copied-target message
variant) instead of a swallowed success. NC: removing the flush reddens
exactly the two new copy tests.
Lock-path termination:canonicalLockPath recursed with a fresh
per-frame hop bound, so a cycle whose length does not divide 8
(a -> b -> c -> a) recursed forever on the lock path. The recursion now
carries a total budget of 64 hops; when spent, the next frame walks
zero hops and canonicalizes the cycle member it stopped on. NC: the
unbounded form reddens exactly the new 3-cycle test (timeout).
The copy-fallback target flush now opens with "r+" (Windows FlushFileBuffers requires GENERIC_WRITE; a read-only handle could fail
the flush and report a false durability error on published bytes). The
test pins the write-access flag; NC: reverting to "r" reddens exactly
that test. 68/68 green.
Rollback fail-closed on unsupported flush (e5e71aa)
The Persistence Integrity row asked what happens when the flush-access
retry itself is unsupported. The copy+fsync fallback answered by
silently downgrading durability: on a filesystem that rejects the
write-handle flush with EPERM/ENOTSUP/ENOSYS it copied the buffer over
the target and reported success. The fallback is removed: those three
codes now raise UnsupportedFilesystemError (target path and cause
attached) and the write fails closed - a filesystem that cannot prove
the flush cannot prove the publish. The five fallback tests collapse to
one fail-closed test that drives all three codes through the same
publish; negative control: re-adding the copy fallback reddens exactly
that test. 66/66 green at this head.
Security Boundaries disposition (already-registered family). The row
repeats the ancestor-swap claim (openat/renameat-style directory-identity
pinning) already dispositioned and registered: the fix lands once at the
publish-primitive owner and copies reconcile at rebase (record 6106959976
on issue 1408; tracking 1991). No code churn this round for that row.

New Features
File reads now indicate when returned content is clipped or incomplete, including line-based and indentation reads.
File writes check previously observed versions before updating. Creating a file requires the target to be absent unless it was previously observed; edits require an observation.
Bug Fixes
Writes to the same file are serialized and reject paths outside the task workspace, changed targets, and unsafe symbolic-link paths.
JSON writes through symbolic links use the resolved target for locking and updates, preserve existing permissions, and better handle save and cleanup failures.
The changes add per-task file observations and guarded writes, plus shared text-publication and canonical-lock-path utilities. Reads record matching file versions and completeness. Writes validate observations, workspace paths, and cancellation before publishing. JSON writes use resolved targets and delegate publication to safeWriteText.
Changes: File observation and write safety
Layer / File(s)
Summary
Record read observations and completeness src/core/task/observationRegistry.ts, src/core/task/Task.ts, src/core/tools/ReadFileTool.ts, src/integrations/misc/indentation-reader.ts, src/core/task/__tests__/*, src/core/tools/__tests__/readFileTool.spec.ts, src/integrations/misc/__tests__/*
Tasks now own observation registries. Native and legacy reads record matching version tokens, and completeness reflects clipping, truncation, partial views, and lossy decoding. Indentation-reader results distinguish clipped lines from omitted lines.
Resolve, stage, and publish files src/services/file-safety/safeWriteText.ts, src/services/file-safety/__tests__/safeWriteText.spec.ts, src/utils/fileLock.ts, src/utils/__tests__/fileLock.spec.ts
safeWriteText resolves targets, stages and syncs content, and supports backups, no-replace publication, and target and ancestor checks. fileLock uses canonical lock paths while preserving caller paths for operations.
Guarded writes serialize by path and apply create, edit, and version-check rules. They validate workspace containment and cancellation, publish under a resolved-path lock, and refresh observations after successful writes.
Use resolved targets for JSON writes src/utils/safeWriteJson.ts, src/utils/__tests__/safeWriteJson*.spec.ts, src/eslint-suppressions.json
safeWriteJson uses resolved lock and publish targets, reads and merges JSON at the resolved target, and delegates publication and backup handling to safeWriteText.

Concurrent writes through aliases may need a read and retry, and an edit followed by a durability error may need its observation refreshed. These bounded risks warrant owner awareness but do not presently block merging.
Security Architecture Review
Security architecture risk:🟡 Moderate · up to bce5c
A filesystem error can occur after saved permissions have changed, while previously allowed actions remain authorized in memory until a refresh. This conditional risk requires existing authorization and a publication failure; no new remote access or privilege escalation was demonstrated.
Retained concerns
Medium · security · inferred: The newly introduced committed-but-rejected JSON outcome is not reconciled by MCP permission updates. A POSIX directory-sync failure can occur after the configuration rename; the rejection skips live tool-permission refresh while programmatic-update suppression can discard watcher events. Removing an existing alwaysAllow grant can therefore leave the prior in-memory grant available to automatic approval until another refresh. This requires an existing grant, enabled MCP automatic approval, and the filesystem failure; actual tool execution under this condition was not demonstrated.
Security review details
Security Blast Radius
inferred — The material exposure is local filesystem state and authorization derived from configuration written through the shared JSON utility. MCP updates can affect global or project configuration; task-history persistence also inherits the changed failure contract. The available evidence does not establish cross-tenant exposure or additional operating-system privileges.
Security Findings and Attack Paths
inferred — A model-requested MCP tool could remain eligible for automatic approval after a saved grant removal if directory synchronization fails after commit and the live permission refresh is skipped. The approval decision consults the supplied server-tool flags and additionally requires global MCP automatic approval. No evidence establishes attacker control of the filesystem failure or demonstrates execution through this conditional path.
Trust Boundaries and Controls
observed — Read observations follow ignore checks and user approval. Within the new guard, observations authorize mutation scope and detect stale versions; they are not a replacement for path-access authorization or protection against non-cooperating filesystem writers. Existing model-write publication remains outside this new guard boundary.
Resilience and Maintainability Implications
observed — With backup mode enabled, a post-commit directory-sync failure leaves new content at the destination and old content at a backup path, because backup deletion is success-only and rollback is pre-commit-only. JSON cleanup does not remove that backup. Backup retention after an unlink failure already existed at the base; this PR adds another retention path without establishing broader read permissions.
Hardening Proposals
proposed — Handle committed-but-not-durable outcomes explicitly at security-sensitive callers: reconcile live permissions with the published configuration or suspend automatic approval until reconciliation completes. Give retained backups explicit cleanup and recovery ownership without rolling old content over an already committed update.

Explanation
The description covers the linked issues, implementation, rationale, review focus, test procedure, environment, checklist, documentation impact, and additional notes. It is detailed and aligned with the pull request changes.
Full details: Linked Issues check
Explanation
Issue #2020 requirements are met. guardedWrite selects create-if-absent and version-token replacement guards from ObservationRegistry. Writes to one resolved path use a FIFO chain. Cancellation is re-checked when a queued link runs and before publication. Guard rejection returns read-first or re-read-then-retry remediation. Tests cover these behaviors, including concurrency and cancellation cases.
Full details: Out of Scope Changes check
Explanation
The supporting changes have a direct connection to issue #2020. ReadFileTool, ObservationRegistry, and indentation metadata provide the observations and completeness required for guard selection. safeWriteText, fileLock, Task, and related tests provide containment, publication, locking, and cancellation behavior used by guardedWrite. The safeWriteJson and durability changes preserve shared write-path behavior. No demonstrated unrelated product change appears in the reviewed scope.
Full details: Regression Evidence
Explanation
Focused coverage is present at the lowest valid layers. guardedWrite.spec.ts covers guard selection, stale and missing files, cancellation, FIFO ordering, containment and symlink races, lock serialization, completeness, refresh, and error propagation. safeWriteText.spec.ts covers staging, backup, cleanup, durability failures, no-replace commits, symlinks, ancestor replacement, and resolution errors. readFileTool.spec.ts and indentation-reader tests cover stat races, lossy decoding, clipping, truncation, partial reads, and native/legacy paths. Registry and Task cancellation tests cover their new state. No durable UI change requires a Playwright snapshot.
Full details: Persistence Integrity
Explanation
No changed persistence path meets the failure condition. safeWriteText writes to a staged file, fsyncs it, and commits with rename or atomic no-replace link; it awaits asynchronous filesystem operations and reports post-commit durability and backup-cleanup failures. safeWriteJson awaits streaming completion, then safeWriteText fsyncs and publishes the staged file under the shared lock. Failed pre-commit writes preserve the target and clean or report partial backups. No persisted default is introduced without propagation.
Full details: Security Boundaries
Explanation
The changed guarded publish path still bypasses the workspace allowlist under a pathname race. src/core/tools/guardedWrite.ts:526-555 records containment and ancestor identities, and src/services/file-safety/safeWriteText.ts:593-632 rechecks them. The commit then uses pathname-based fs.link or fs.rename at :706-733. Between the final realpath/stat checks and that syscall, a local process can move an authorized parent outside the workspace and replace its old name with a symlink to the moved directory. The staging and target pathnames then resolve through the symlink, so the write can create or replace a file outside the workspace. The implementation explicitly documents this remaining window at safeWriteText.ts:59-65.
Resolution
Use no-follow directory handles and descriptor-relative publication. Open and pin the workspace and target-parent directories with symlink rejection, validate identities through those handles, and commit relative to the pinned parent handle with descriptor-relative no-replace or rename operations. Reject missing or changed parent components. Add a regression test that swaps an authorized parent for a symlink to a moved outside directory between final validation and publication, and verify that no outside file changes.
Full details: Lifecycle Resource Cleanup
Explanation
A changed Windows DACL cleanup path can leak dump files. In safeWriteText.ts, a failed icacls /save creates dumpPath but line 585 unlinks it with .catch(() => {}) and does not retain the path. If fs.unlink returns EPERM or EBUSY, the partial .safeWriteText.acl_* file remains while the write continues. A successful save has the same risk at lines 806-810: the final unlink error is swallowed, including after a successful publish. The added tests only verify that unlink is attempted; they do not cover unlink failure.
Resolution
Retain every DACL dump path, including paths from failed saves. Retry cleanup for both failed-save and final cleanup paths. If cleanup still fails, surface the orphan path in a dedicated error or persist/enqueue reliable cleanup. Add Windows tests where DACL dump unlink fails in both paths.

Learning: In src/services/file-safety/safeWriteText.ts, safeWriteText intentionally treats all post-commit parent-directory fsync failures, including EINVAL and EBADF, as PostCommitDurabilityError. A successful return must not imply durability that the filesystem did not grant. The error identifies the target where committed content remains, while callers decide how to handle retries. Do not recommend silently tolerating unsupported directory-fsync errors.

src/core/tools/guardedWrite.ts: 2
649-664: The PostCommitDurabilityError observation refresh does not cover the edit branch.
This try/catch wraps only the create and update branch. The edit branch (Lines 581-598) calls replaceIfVersion outside it. If that call throws PostCommitDurabilityError, the content is already published at absolutePath. The observation keeps the old token. The next write by the same task is then rejected as stale, even though the earlier write landed.
To fix this, move the handler around all publish branches. Set staysPartial = obs.complete === false before the awaited edit publish. The handler can then keep partial completeness for an edit.
An earlier review flagged this gap, and it is still open in the supplied code.

465-479: A post-commit durability failure still restores the backup over the committed content.
The issue from the earlier review is still present in this revision.
Line 409 renames tempPath onto targetPath. The new content is now published.
If the parent-directory fsync then fails, Line 428 throws PostCommitDurabilityError.
When backup: true, backupPath && releaseBackupOnSuccess is still true at Line 466.
Line 468 then renames backupPath back onto targetPath. The old content replaces the new content.
The function still throws PostCommitDurabilityError, whose message says the content is at the target. That statement is false at this point.
safeWriteJson always passes backup: true (src/utils/safeWriteJson.ts, around Lines 123-131), so every JSON write can reach this path. The test at src/services/file-safety/__tests__/safeWriteText.spec.ts Lines 869-887 runs without backup, so it does not cover this case.
Fix:
Set a committed flag right after the Step 4 rename.
If committed is true, do not restore the backup. Remove the backup on a best-effort basis, then rethrow.
Add a backup: true test for this path.
🐛 Proposed fix
let releaseBackupOnSuccess = false
+ let committed = false
…-code gate
guardedWrite.ts is wired into the tools by a later unit of this chain, so the production dead-code check reports the whole module as unused on this intermediate branch. The ignore entry is removed when that unit lands.
… paths
The baseline refactor replaced the abortTask/dispose bodies with requestAbort/requestDispose, so the branch's monotonic cancellation counter stopped moving and the S4a guarded-write fence could publish a cancelled run's write. Advance it in both central paths.
The resume test now matches the run-state model: the legacy abort flag is no longer cleared on resume, which is precisely why the generation exists.

Learning: In src/services/file-safety/safeWriteText.ts, error-path cleanup must unlink tempPath only while the write still owns the staging path. After publication and staging-name consumption, stagingCommitted prevents later errors from unlinking a path reused by another write. Pre-publication refusals must still clean up the staged file and the write's own staging directory.

The check asks for no-follow directory handles plus descriptor-relative publication. The series already pins directory identity and re-verifies it at publish time; the remaining gap is publishing relative to an open directory handle, which Node core cannot express (openat/renameat/fstatat are not exposed).
What is already in this PR:
the containment walk pins every directory it walked with its (dev, ino) identity (src/core/tools/guardedWrite.ts, walk around line 409);
those identities are re-checked immediately before publish (around line 544);
the authorized resolved path is threaded through as expectedResolvedPath, so a re-resolution that lands elsewhere is fatal, not silent.
The unsolved part (fd-relative publish) needs a design decision between a Linux /proc/self/fd path, a native openat/renameat addon, or an explicit documented residual-risk decision. That decision belongs in a follow-up, so I filed #2148 (guardedWrite: evaluate descriptor-relative publication for containment races) with the threat model, current mitigations, options and acceptance criteria.
This PR therefore keeps the pinned-identity behavior and does not grow a platform-specific publish path that could not be reviewed or shipped in a split unit.
The other two findings are being fixed in the PR: the post-commit backup cleanup will retry and then surface an OrphanedBackupError instead of returning success, and fileLock.spec.ts gets the realpath-error regression tests the checklist asks for.
…ileLock realpath fallbacks
Lifecycle Resource Cleanup (pre-merge check): safeWriteText returned success after silently abandoning the pre-write backup when its post-commit unlink failed (EPERM/EBUSY on Windows). The cleanup now retries once and, if the copy is still on disk, throws OrphanedPublishedBackupError carrying the published path, the orphaned backup path and the cleanup error, instead of reporting success.
Regression Evidence (pre-merge check): fileLock.spec now covers realpath failing with EACCES and with a code-less error, asserting that lock acquisition falls back to the caller's lexical path and that withFileLock hands the operation the caller's own path.
Security Boundaries: the remaining descriptor-relative publication gap is tracked in Zoo-Code-Org#2148 (filed with threat model, current pinned-identity mitigations, options and acceptance criteria); this PR keeps the pinned (dev, ino) re-check and expectedResolvedPath behavior.

…ity failure and share the read observation helpers
Review threads on Zoo-Code-Org#1914:
- PostCommitDurabilityError is thrown after the content reached the target, so guardedWrite now catches it, records the token that is on disk (keeping the observation's completeness), and rethrows: a later write by this task no longer fails stale against the token that was replaced.
- The native and legacy ReadFileTool paths now call one recordObservation helper (plus statForObservation and decodeReadText) instead of duplicating the TOCTOU pre/post-token invariant.
- canonicalizeNearestAncestor fails closed on realpath errors other than ENOENT/ENOTDIR instead of returning a lexical key that cannot match the guard's pin.
- The cancellation counter comments in Task.ts and guardedWrite.ts describe the generation as an independent fence advanced by requestAbort()/requestDispose() (resume no longer clears the abort flag).
Local: eslint/tsc clean; ReadFileTool + guardedWrite + file-safety + fileLock + Task suites 444 passed.
…p as logged, not failed
safeWriteJson documents backup deletion as best-effort for its callers (the write commits, the pre-write copy may be left behind, the call still succeeds). The new OrphanedPublishedBackupError signal from safeWriteText propagated through that caller and turned ten safeWriteJson tests red on the platform lanes, so safeWriteJson now catches exactly that error, logs the orphan path as a warning, and keeps its contract. Callers that use safeWriteText directly (the guarded/edit path) still receive the strict signal.
Local evidence: safeWriteJson + safeWriteText suites 94 passed / 1 skipped in this worktree. (Local eslint/tsc are not usable in this worktree because its borrowed node_modules layout cannot resolve the workspace packages; CI compile is the authority.)
The Security Boundaries row asks for the same descriptor-relative publication primitive scoped in #2148. The Windows DACL dump-path row is a cleanup-hardening item on the same publish primitive.
No new review threads are open on this PR. The remaining rows describe design-level file-safety primitives (descriptor-relative, no-follow publication; secret-free view-local persistence) that are being resolved in the tracked design issues above rather than by widening this unit's scope.

This PR adds
guardedWrite, the single publish entry point every write tool will use.

Description
What this PR changes:
guardedWrite(): resolves the target, checks containment, picks the guard from the task'sObservationRegistry- create-if-absent, recreate, compare-and-swap on the version token, or the unobserved-edit remediation - and runs the publish on the per-path FIFO chain.createIfAbsentandreplaceIfVersionsemantics: a stale version token or an unobserved overwrite is refused instead of clobbering the file. Why the diff looks bigger than the change: the chain is non-cumulative - this branch is based on
mainand carries its own copy of the earlier units' content, which the GitHub diff shows as context.Review focus:

Re-review round (b95efab)
Two Minor findings fixed with regression tests and negative controls:
inode at the target that nothing fsynced (the staged inode's pre-commit
fsync does not cover it; step 4b flushes only the parent directory).
The copied target is now opened and fsynced right after the fallback;
failure reports
PostCommitDurabilityError(new copied-target messagevariant) instead of a swallowed success. NC: removing the flush reddens
exactly the two new copy tests.
canonicalLockPathrecursed with a freshper-frame hop bound, so a cycle whose length does not divide 8
(a -> b -> c -> a) recursed forever on the lock path. The recursion now
carries a total budget of 64 hops; when spent, the next frame walks
zero hops and canonicalizes the cycle member it stopped on. NC: the
unbounded form reddens exactly the new 3-cycle test (timeout).
Windows flush-access fix (a75f05c)
The copy-fallback target flush now opens with
"r+"(WindowsFlushFileBuffersrequiresGENERIC_WRITE; a read-only handle could failthe flush and report a false durability error on published bytes). The
test pins the write-access flag; NC: reverting to
"r"reddens exactlythat test.
The fallback is removed: those three
codes now raise
UnsupportedFilesystemError(target path and causeattached) and the write fails closed - a filesystem that cannot prove
the flush cannot prove the publish.
