feat(task): generate a session title from the first message - #2334
This PR adds an automatic title for new root tasks.
src/core/task/session-title.ts is new. It sends one single-shot completion with a 15-second limit. It removes <system-reminder> blocks from the user message first. If the model fails or returns nothing usable, it uses the first 42 characters of the message as the title.
Task.startTask starts the generation after the first message is stored. The call is fire-and-forget, so the task never waits for it. Only root tasks start it. Resumed tasks and subtasks keep their names.
ClineProvider.applySessionTitle writes the title inside the atomicReadAndUpdate lock. It writes only if the item has no title. A manual rename that lands during generation always wins. The title write is retried up to 3 times on a transient failure.
Please check these points with care:
updateTaskHistory still does not own the title field. Only the rename path and applySessionTitle write it. I updated the comment to say this.
A conflict is signaled with SessionTitleConflictError thrown inside the lock. This avoids a disk write of an unchanged record.
Test Procedure
Automated tests (from src):
npx vitest run core/task/__tests__/session-title.spec.ts core/task/__tests__/Task.session-title.spec.ts core/webview/__tests__/ClineProvider.taskHistory.spec.ts
Result: 3 files, 84 tests, all pass.
The tests cover:
Sanitizing of the user message, the LLM title, and the fallback title.
The timeout and the retry of the title write.
The trigger rules: root task starts it, resume and subtask do not.
applySessionTitle: applied, conflict with an existing title, and a blank title.
Manual steps: start a new task with a long first message. Check that the task list shows a short title. Rename a task while the title generates and check that the manual name stays.
Pre-Submission Checklist
Issue Linked: This PR is linked to an approved GitHub Issue (see "Related GitHub Issue" above).
Scope: My changes are focused on the linked issue (one major feature/fix per PR).
Self-Review: I reviewed my own code with care.
Writing Style: The PR title and description use Simplified Technical English (ASD-STE100): short sentences, plain words, active voice, one idea per sentence, no contractions.
Testing: New and/or updated tests have been added to cover my changes (if applicable).
Test Helpers (test changes only): Shared test helpers (src/test-utils/, @/utils/test-utils) are used only for mechanical duplication; test intent and scenario-specific mocks remain visible.
Visual Snapshot (UI changes only): Not applicable. This PR has no UI change.
Documentation Impact: I checked if my changes need documentation updates (see "Documentation Updates" section below).
Untitled root sessions can receive an automatically generated title based on the initial message. If generation is unavailable or fails, a concise title may be derived from the message instead.
System-reminder content is excluded from the title source.
Bug Fixes
Existing session titles are preserved when automatic title generation runs, and title-generation failures do not interrupt session startup.
Title generation runs in the background and stops when a session is renamed or deleted.
Eligible new root tasks can generate a title in the background from the first user message. The code sanitizes the message, requests a title with a timeout, uses a message-based fallback when needed, and stores the title only if no title already exists.

setTitleIfUnset writes a title only when the task-history record has no title. applySessionTitle trims the title and updates the cache and launched view after a successful write. Tests cover conflicts, cross-instance changes, and broadcast behavior.
Title generation and fallback src/core/task/session-title.ts, src/core/task/__tests__/session-title.spec.ts
Title generation sanitizes the message and requests a 5–10-word title with a 15-second timeout. It uses a message fallback when needed, retries write failures up to three times, and deduplicates concurrent generation for a task. Tests cover sanitization, parsing, fallback, timeout, retries, conflicts, and concurrent generation.
startTask launches generation without waiting when a root task has an initial message, a provider, and a non-Test extension mode. Tests cover excluded task types, extension modes, pending generation, and generation failures.

Explanation
No changed path shows a concrete security-boundary violation. Title generation uses the task’s configured provider, and title writes do not execute model output. The existing task creation path checks the organization allowlist.
Full details: Persistence Integrity
Explanation
The changed title write uses a locked read-modify-write and preserves other persisted fields. It checks for deleted or titled records before writing. Write failures are handled by bounded retries, and task startup does not depend on title generation.
Full details: Linked Issues check
Explanation
[#2333] Background generation, the 15-second timeout, root-task trigger rules, locked title writes, and related tests support the issue. Two fallback requirements remain unmet. Sanitization trims the message before the 42-character slice: 40 spaces followed by ABCDEF produces ABCDEF, not the first 42 characters. Also, applyTitleWithRetry gives up after four failed writes without attempting the fallback title.
Resolution
Select the fallback from the first 42 characters before trimming or collapsing whitespace. Add a leading-whitespace boundary test. After write retries fail, attempt the fallback title and test that path.
Full details: Regression Evidence
Explanation
Generated-title persistence lacks focused coverage. setTitleIfUnset adds a disk write, but its success test in TaskHistoryStore.crossInstance.spec.ts:111-117 checks only the writer’s cache. The provider tests mock safeWriteJson and also check the cache. A title lost after reload could pass these tests. The startup-ordering test now covers the earlier ordering concern.
Resolution
Add a store-level test that applies a title, reloads the history from disk, and verifies that the title and original task persist.
Full details: Lifecycle Resource Cleanup
Explanation
The prior cleanup failure remains. The new title timeout in session-title.ts:168-195 returns after aborting, but singleCompletionHandler disposes its handler only after completePrompt settles. If VS Code LM stalls in model selection, its configuration listener remains registered and repeated tasks can accumulate listeners. OpenAiHandler.completePrompt also ignores the abort signal, so its request can continue after timeout or cancellation.
Resolution
Dispose the completion handler when title generation ends, even if completion is pending. Propagate cancellation to provider requests and cover stalled completion and cleanup paths.

Resolve a mutation gap
Add or strengthen a focused test that fails under the mutation. If the mutant is equivalent, request maintainer approval for the narrowest mutator-specific exclusion and explain why it cannot change behavior:
// Stryker disable next-line ConditionalExpression: normalized input cannot reach the alternate branch
const result = condition ? value : fallback
Broad all exclusions and exclusions without a concrete reason are reported as advisories.

src/core/task-persistence/TaskHistoryStore.ts: 1
1009-1041: Clearing a manual title during generation lets the generated title overwrite it.
renameTaskWithId stores title: undefined when the user clears a title. setTitleIfUnset treats both the cache and the disk record as unset when title is falsy. A pending generation then writes its title over the user's explicit clear. The PR objective states that a manual rename takes precedence. A clear is also a manual action, and the original prompt display is lost until the next rename.
This is a narrow race. It needs a user clear that lands between task start and the end of the 15-second generation window. The title is only a display name and the user can clear it again.
Possible fixes:
Cancel generation for the task when renameTaskWithId runs.
Track a "title manually set" marker, or a generation epoch, and check it inside the lock.
This matches an earlier review comment on ClineProvider.ts. It is repeated here because the check lives in this method.
Abort the locked title write when the on-disk record is missing or already
titled, cut the fallback at 42 characters before collapsing whitespace, and
cover the deleted-task and pending-say startup paths with tests.

74-93: Fix: a deleted task leaves the in-flight map entry and stale cancel state is safe, but one cancellation gap remains.
cancelSessionTitle only affects generations already registered in inFlightTitles. generateSessionTitle registers the entry at line 117 after the first await-free checks, so this is synchronous and safe. The remaining gap is the sequence: renameTaskWithId calls cancelSessionTitle(id) before the generation exists. This cannot occur, because startTask triggers generation after the history item exists and user renames need a listed task.
The earlier concern on clearing is also resolved by the abort path. No defect found in the changed lines.

Description
This PR adds an automatic title for new root tasks.
src/core/task/session-title.tsis new. It sends one single-shot completion with a 15-second limit. It removes<system-reminder>blocks from the user message first. If the model fails or returns nothing usable, it uses the first 42 characters of the message as the title.Task.startTaskstarts the generation after the first message is stored. The call is fire-and-forget, so the task never waits for it. Only root tasks start it. Resumed tasks and subtasks keep their names.ClineProvider.applySessionTitlewrites the title inside theatomicReadAndUpdatelock. It writes only if the item has no title. A manual rename that lands during generation always wins. The title write is retried up to 3 times on a transient failure. Please check these points with care:
updateTaskHistorystill does not own thetitlefield. Only the rename path andapplySessionTitlewrite it. I updated the comment to say this.SessionTitleConflictErrorthrown inside the lock. This avoids a disk write of an unchanged record.Test Procedure
Automated tests (from
src):npx vitest run core/task/__tests__/session-title.spec.ts core/task/__tests__/Task.session-title.spec.ts core/webview/__tests__/ClineProvider.taskHistory.spec.tsResult: 3 files, 84 tests, all pass.
The tests cover:
applySessionTitle: applied, conflict with an existing title, and a blank title.Manual steps: start a new task with a long first message. Check that the task list shows a short title. Rename a task while the title generates and check that the manual name stays.
